Yes, there are patient intake forms that we ask you to fill out prior to the time of your appointment with us. These patient intake forms will be sent to you virtually via the email you provide us with when you make an appointment with us.
These forms should only take a few minutes to fill out and cover your medical history as well as any problems you may be experiencing and would like to receive treatment for. Please be as detailed as possible when filling out this information, as it provides our clinicians with the appropriate information to provide you with a detailed treatment plan.
On the day of your appointment with us, we ask that you bring a physical ID and insurance card as well as any medical records or diagnostic imaging you feel is necessary to share with your clinician. We will also provide you with a list of what to bring with you prior to the time of your appointment.
Because it is likely you will be moving your body throughout your appointment with us, we ask that you wear loose-fitting and comfortable clothing.
Concierge Health & Wellness is happy to accept worker’s compensation, Medicare, and coverage from most major health insurance plans. We ask that you verify coverage for your services with us with your insurance provider directly prior to scheduling an appointment with us.
If you choose to use your insurance for physical therapy services, the majority of private insurance in Montana does not require a referral. however, it is best for you to reach out to your insurance carrier to verify your individual benefits. otherwise, if you choose not to utilize insurance, no referral is required for treatment at Concierge Health & Wellness.
