Please do not use this form if you have an urgent medical problem or you need to reschedule an existing appointment. Instead, contact our office at (406) 707-6082. After you submit your request, our appointment scheduler will respond within 48 hours. Patient forms will be sent to you via email, at which time you may fill them out online prior to your appointment time.
Medical Emergencies
In the event of a life-threatening medical emergency, call emergency medical services at 911 immediately. DO NOT WAIT! Inform the emergency room staff that you are a patient of Concierge Health & Wellness, and they will contact us.
Insurance / Payment / Billing
At Concierge Health & Wellness, we accept worker’s compensation, Medicare, and most major insurance plans. Please contact your insurance company, or you may call our office at (406) 707-6082 to find out if we accept your plan.
If your insurance plan requires you to pay a co-payment, co-insurance, and/or a deductible, you will need to pay at the time of your visit. For your convenience, we accept cash, checks, Visa, MasterCard, Discover, and American Express.

Referrals
For your convenience, Concierge Health & Wellness does not require a referral from your primary care physician to receive services from our dedicated team of licensed physical therapy clinicians. However, it is important to note that certain government payers require a referral including Medicare, Tricare, VA, etc. To confirm whether or not you may need a referral, please contact our office.

Should you need to cancel your appointment with us, we ask that you provide at least 24 hours’ notice to avoid a $30 no-show fee.
